Office Policies

Your Appointment 
To provide the best service to all patients, we ask that you arrive 15 minutes early to check-in and complete any  necessary paperwork. When checking in, please let our front desk staff know of any changes in address, phone number, insurance, etc., since your last visit.  

 Please bring with you:
  -valid photo ID
  -current insurance card(s)/financial assistance approval letter
  -any paperwork we've asked you to fill out
  -any scans, CTs, MRIs, previous records, etc, that may help in diagnoising you
  -co-pay: you may be asked to reschedule if you cannot meet your co-pay 
   requirements at time of service

 Your initial Allergy appointment may take up to 2 hours
  -allergy testing is tailored to the individual which largely affects the length of each appointment
Late Appointments
Our office has a 15 minute late policy. To avoid being asked to reschedule, please contact our office if you think you will be late for your appointment: 614-366-3687.

As a courtesy to our office and other patients, we prefer 24 hrs. notice when rescheduling your appointmnet. To reschedule your appointment, please call our scheduling line  at 614-366-3687.

Call Backs
Please be aware that when our physicians are in clinic, so are their assistants.
When leaving a message or sending an email during your physician's clinic hours, 
please allow up to 24 hours for a return phone call. 
If it is an emergency, please call 911 or go to the nearest emergency room.
